New research provides recommendations to help manage neuropathic pain
LONDON, ONTATIO – Researchers at Lawson Health Research Institute are the first in Canada to develop clinical practice guidelines for managing neuropathic pain with patients who have experienced a spinal cord injury (SCI).
Neuropathic pain is complex and chronic, and is the most common complication reported by people following SCI. The research team worked with care providers at Parkwood Institute, part of the St. Joseph’s Health Care London family, and an international panel to address the complex and unique challenges for managing pain during recovery and rehabilitation.

A spinal cord injury (SCI) is damage anywhere along the spinal cord, often due to an accident or other trauma. SCI typically causes a loss of movement and feeling below the damaged part of the spinal cord, often leading to paralysis and other changes in functioning. People with SCI may be more likely to develop depressed mood than members of the general population: Current research shows that up to 25 percent of people with SCI experience depression, and up to 12 percent report major clinical depression.

TRAVELLING over 4,000 miles from her home town in Northern Ireland to Project Walk in Longwood, Orlando, bubbly Jennifer Smyth is on an epic journey, not to accumulate the rich life experiences of adventurous travel, but rather to regain her legs – the use of which she lost in a catastrophic gymnastic accident almost three years ago.
She explained: “Ever since I was a little girl I have been consumed by gymnastics and have devoted myself to the discipline of athletes, always pushing myself to be the best I can be. I don’t know any other way to live. The accident happened on a Tuesday evening after school, I was on my last vault before moving to the next event, and when I landed I just couldn’t move.
The body is a series of checks and balances. This is true of muscles that push and countering muscles that pull. It is also true of the nervous system that operates in a balancing type of process. Individuals with higher level spinal cord injury can develop a complication called Autonomic Dysreflexia (AD). This is a condition where the sympathetic nervous system is left unchecked by the parasympathetic nervous system.
There is a bundle of nerves at the Thoracic vertebrae number six (T6) level that is a major junction where nerves come close together in the spinal cord. Individuals with spinal cord injury above this level have a disruption in the nerve segment. For these individuals, stimulation of the body at or below T6 can send confusing messages to the brain as the message will create a huge discharge of the sympathetic nervous system using all of the blood flow in the abdomen without the counter control of the parasympathetic nervous system to contain it. Blood pressure rises to extremely high proportions.
A new patient study has paved the way for a new opportunity to rehabilitate patients with spinal cord damage.
Dr Anastasia Shulga led the Helsinki University Hospital study in which two patients with spinal cord injuries received a form of treatment that combined transcranial magnetic stimulation with simultaneous peripheral nerve stimulation given repeatedly for nearly six months.
This was the first time that attempts were made to rehabilitate patients paralysed as a result of a spinal cord injury through long-term stimulation treatment of this type.
